Business Account Application

Personal guarantee. In consideration of Mid Mo Strap extending Net-30 credit to the business named in this application (the Applicant), the undersigned individually and personally, unconditionally and irrevocably, guarantees the full and prompt payment of all amounts the Applicant now owes or later owes to Mid Mo Strap, including all charges on the account together with any interest, late charges, and reasonable costs of collection including attorney fees and court costs. This is a continuing guaranty of payment and not merely of collection, and it remains in effect until all amounts owed are paid in full. The undersigned waives notice of acceptance of this guaranty and notice of any change to the account or credit terms, and agrees that Mid Mo Strap need not first proceed against the Applicant before enforcing this guaranty. The undersigned represents that they are an owner or authorized officer of the Applicant and are authorized to bind both the Applicant and themselves individually. Typing your full legal name below is your electronic signature and has the same legal effect as a handwritten signature.

Credit-check authorization. The undersigned authorizes Mid Mo Strap and its agents to investigate the credit history of the Applicant business and to obtain business credit reports and related information from Experian, Dun and Bradstreet, Equifax, and other business credit reporting agencies and trade references, both to evaluate this application and, on a continuing basis, to review, update, renew, or collect on the account for as long as credit is extended. The undersigned authorizes any person, bank, or agency contacted to release the requested credit information to Mid Mo Strap. A photocopy or electronic copy of this authorization is as valid as the original. Typing your full legal name below is your electronic signature.
